ASSUMPTION OF RISK AND RELEASE OF LIABILITY I, the undersigned, acknowledge and agree that participation in any Falcon’s Edge Power Skating programs, training sessions, camps, or events (hereinafter referred to as “Activities”) involves inherent risks, including but not limited to, risks of serious bodily injury, permanent disability, paralysis, or death. I knowingly and freely assume all such risks, whether known or unknown, and assume full responsibility for my participation. I further acknowledge that Falcon’s Edge Power Skating, its affiliated and associated companies, officers, directors, coaches, agents, employees, volunteers, and sponsors (collectively, the “Released Parties”) are not responsible for injuries, damages, losses, or claims of any nature, whether arising from negligence or otherwise. To the fullest extent permitted by law, I, on behalf of myself (or my minor child), my heirs, executors, administrators, and assigns, do hereby waive, release, discharge, and covenant not to sue the Released Parties from any and all claims, liabilities, or demands of any nature whatsoever that may arise out of or in connection with my (or my minor child’s) participation in Activities, including any claims related communicable diseases. 2. MEDICAL CONSENT AND EMERGENCY TREATMENT I certify that I (or my minor child) am physically fit and able to participate in the Activities. I consent to emergency medical treatment deemed necessary for me (or my minor child) in case of an accident, illness, or other emergency, and I accept full financial responsibility for any medical expenses incurred. 3. PHOTO, VIDEO, AND MEDIA RELEASE I understand and agree that Falcon’s Edge Power Skating and its authorized representatives may take photographs, video recordings, and audio recordings of participants during Activities. I hereby grant Falcon’s Edge Power Skating and its affiliated or associated companies, directors, officers, agents, employees, customers, and appointed advertising agencies an irrevocable, royalty-free, perpetual, worldwide right to use, reproduce, modify, publish, distribute, and publicly display my (or my minor child’s) image, likeness, voice, or name in promotional materials, advertising, social media, or any other business-related purposes. I waive any rights to inspect or approve the finished media and release all claims related to privacy, defamation, or financial compensation in connection with the use of these materials. 4. INDEMNIFICATION I agree to indemnify and hold harmless the Released Parties from any and all claims, demands, liabilities, damages, or expenses (including attorneys fees) arising from my (or my minor child’s) participation in Activities. 5. ACKNOWLEDGMENT AND SIGNATURE I HAVE READ AND FULLY UNDERSTAND THIS LIABILITY WAIVER AND MEDIA RELEASE FORM.
